Us Helping Us is a rapidly growing and mission-driven nonprofit. For the past 30 years, we have provided innovative programs and services to improve the health and wellbeing of racial, sexual orientation and gender minorities, people living with HIV/AIDS, and persons with substance abuse disorders and mental illness. As it exists today, we currently operate two office locations in the greater Washington, DC area and offer a variety of clinical, behavioral/mental health and social support services in a trusting, compassionate and non-judgmental environment. We are currently hiring for a Youth Empowerment Specialist and are looking for an energetic, independently motivated and committed person to fill this position.



The Non-Medical Case Manager will serve our very diverse client population in the greater Washington, DC area (Washington, DC; Prince George's and Montgomery Counties, MD; and Arlington and Fairfax Counties, VA) via in-office visits and off-site via telemedicine technologies. Our clients present with a variety of LGBTQ concerns, depression, anxiety, disordered eating/body image concerns, substance abuse, trauma, and aging/life transition concerns. The Non-Medical Case Manager will have responsibilities ranging from direct services (individualized treatment plan development to address emergent needs) to program development (facilitate support groups with marginalized populations such those vulnerable for or living with HIV).

The Non-Medical Case Manager will have the following responsibilities:

  • Assess clients’ needs, situations, and support networks to enable them to achieve their medical goals as well as to improve and/or maintain their current health situation, especially LGBTQ youth and adults.

  • Develop Individualized Health Plans to improve clients’ overall health and well-being, especially people living with or at significant risk for HIV/AIDS.

  • Maintain confidential up-to-date client contact and fulfillment of treatment goals and help clients adjust to changes and challenges in their lives such as medical diagnosis or illness, divorce, or unemployment.

  • Advocate for and refer clients to community resources (Medical Case Management, emergency food assistance, PrEP) that will improve clients' overall health and well-being.

  • Respond to crisis situations, such as intimate partner violence and abuse, and be knowledge of available community resources and/or evidence-based treatments.

  • Work to build the trust and confidence of clients to overcome any fears or anxiety related to their health condition(s) or with navigating the healthcare system.

  • Assess clients’ essential social support needs, life situations, and support networks to understand how these factors may impact their engagement in medical care.

  • Work closely with Community Health Workers to implement Individualized Treatment Plans for clients and to identify services to address other aspects of their life, such as mental health and substance abuse treatment, domestic violence counseling, housing programs, and education and employment services.

  • Follow-up with clients to ensure they attend their appointments, fill their prescriptions, take their medication(s) as prescribed and/or address any barriers or challenges to adhere to their medication(s), and remain engaged in care, among other things.

  • Meet on a monthly basis with an interdisciplinary team consisting of Community Health Workers, Therapists and other clinical providers to discuss successes, challenges, and innovative approaches to better these core organizational services.

  • Perform other duties as assigned.


Experience and Education

  • Degree in health or human services related field preferred. High School diploma or GED required.

  • Minimum of 2 years of case management experience, HIV case management preferred.

  • Ongoing education/training in HIV/STI and chronic disease related subjects.

  • Agency will provide new hires with training regarding confidentiality, client rights and grievance procedure.

  • This position requires intermediate computer skills required, including proficient use of Microsoft Office Suite and electronic health records.

  • Commitment: Passion for the mission of the organization as well as linkage and retention-in-care services, including a strong commitment to working to improve the health and quality of life of LGBT youth and adults, people living with HIV/AIDS, other chronic diseases, and mental health disorders.

  • Knowledge: Must have demonstrated knowledge of a variety of health-related services in DC, Maryland, and Virginia, along with an interest and ability to expand knowledge through training.

  • Experience: Must demonstrate experience with issues related to HIV care and treatment. Experience with issues that put people at risk for poorer health is also highly helpful, such as homelessness, incarceration, domestic violence, and/or substance abuse.

  • Skills: Strong communication skills are essential. Demonstrated ability to conduct sensitive, empathetic interviews and conversations that respect the dignity and diversity of our clients. Ability to engage staff and clients in a supportive and empowering way. Ability to help manage conflict. Ability to maintain client records and information in an accurate, timely and confidential manner.

  • Personal qualities: Personal qualities including energy, intelligence, sound judgment, ability to work independently (with less supervision); the ability to work effectively in a busy environment; and the interpersonal skills to work effectively in partnership with others.

  • Plan and organize work in an effective and timely manner and, in a team environment, maintain a flexible schedule in accordance with program and organizational needs.


  • Work a minimum of 40 hours per week, including evenings and weekend commitments.

  • The salary for this position depends on experience and qualifications and ranges from $45,000-$60,000. Us Helping Us has a great benefits package with annual leave, federally observed holidays, and health/dental/vision insurance for full-time employees.

  • U.S. citizen or permanent resident of the U.S. with a valid work permit required.

  • Must submit to and successfully complete a background investigation.
